A 60-year-old male patient developed right-sided facial paresis one year ago following a stroke. As a result, he presents with paralysis and ptosis of the upper and lower eyelids, along with asymmetry of the midface and lower face. In addition to the aesthetic concerns, the patient experiences several functional complications, including drooling from the oral commissure, epiphora (excessive tearing), persistent scleral irritation, ectropion, and lagophthalmos. The lower eyelid margin is flaccid.
A two-stage surgical plan has been scheduled for the patient:
First Stage:
- Elevation of the lower eyelid using a spacer graft
- Midface lifting with APTOS Needle 4.0
- Oral commissure elevation using the APTOS Spring Method
- Lower face lifting with APTOS Excellence Visage
Second Stage (Planned):
- Correction of upper eyelid lagophthalmos using an APTOS spacer graft
